Sachs clutch problem

Evening all.
Right well I've recently ( 2months ago) had a sachs performance clutch an lightened smf fitted.
It's all been good up intill three days ago when I've started hearing a knocking sound coming from around the gearbox clutch area, I can also feel it in the clutch peddle if I apply a slight bit of pressure whilst in gear driving. Changing gear past 6k rpm is pretty much impossible as in the clutch doesn't want to disengage. And yesterday while I was at the lights in first gear with Clutch In I reved it to 6k and the car jumped forward with out letting the clutch out?
I'm going to be taking my car back to the garage that fitted it on fri but just wanted to see if any of you lot know what the problem could be.

Sounds like a failed/ing clutch, could be if not done right?

Did you bed it in? Or start ragging it around straight away?

I've heard smf can damage gearbox so hope it's not that!
 
Why on earth are you revving it to 6k stood still at the traffic lights anyway?

Lol it was just to see if the sound speeded up as the revs went up, I didn't just hold it there, I love her to much to blow it up :)
It's sounding more like some thing is loose and wobbling like clutch plate or something?
