Headlamp misting up, likely causes?

Just noticed the other day that my drivers side headlamp is misting up. Seems to happen when we have a cooler night, perhaps after a humid day and it's misted when I come to start the car in the morning. Any suggestions as to likely causes and what needs fixing - I'd been planning to upgrade the bulbs anyway so maybe I can include that in the work.

I've had this problem with both sides of my B7.

Initially, Audi did it under warranty, but it entailed taking it in over a period of several weeks for them to take photographs. They eventually replaced the offside one. I occasionally get condensation in the nearside one, but they claim that this the amount is acceptable and won't do anything about it.

I always had the impression that they knew all about it. It is annoying when people point it out. Shouldn't happen on a car of this value.
